Fraser Health, the Langley Division of Family Practice and the Langley Memorial Hospital Foundation are offering one-time funding in the amount of $10,000.00 to enable local service agencies to increase their participation in the Seniors’ LINC initiative.
We want to partner with non-profit organizations and volunteers that are interested in working to wrap care around seniors. These funds could be used to help organizations to build capacity, for example, by providing more training for staff or volunteers, resources to purchase tools or equipment to support outreach to seniors, or help to with policy or process development.
We recognize that supporting seniors is about providing more than medical care – it is about making sure that seniors have access to the tools and resources they need to live healthy lives in the community.
This grant is open to charities, onhealthy amoxicillin non-profit and not for profit agencies that provide services to seniors in Langley. Funding must be used to increase capacity of the organization(s) to provide service to seniors or to support increased collaboration between providers for the purpose of enhancing care to seniors.

Seniors’ LINC is a partnership between Fraser Health, the Langley Division of Family Practice, and community agencies who are working together to more effectively deliver health services to seniors in Langley. The goals of Seniors’ LINC are to:
- Support frail seniors to remain independent in their homes and live confidently in their community
- Enable seniors to avoid or delay the need for hospital or residential care
- Enhance and strengthen community partnerships to better support frail seniors
